At the Krasnoyarsk Medical University, the admission campaign is in full swing. What changes have taken place in the recruitment of applicants who want to become doctors, how to enter the KrasSMU and what can really solve the personnel problem - says the rector of the Krasnoyarsk State Medical University Ivan Pavlovich Artyukhov.


Ivan Pavlovich, the admission campaign to the medical university is in full swing, this year there are significant differences. Tell us about the process of recruiting future doctors?

The current admission campaign is taking place for the first time during the implementation of the new law on education, which determined target set features, quantitative and quality characteristics, a system of interaction between the employer and the applicant, the employer and the university. The second feature is that we have another failure in the number of applicants. Compared to last year, two thousand fewer eleventh-graders graduate. Our task is to fulfill the state task, which is 61 more budget places than last year, so it is necessary to attract applicants to our university, but at the same time maintain a high level.

If we talk about the target recruitment, then earlier this concept was regulated by twenty years of regulations, the system of interaction between a student and his employer was freely interpreted. The young doctor, in fact and legally, could not go to the place of his appointment, he knew that nothing would happen to him for this, our students often used this, and not a single court could force him to go to work. New law on education and a decree of the government of the Russian Federation determined a different procedure for interaction. How it looks now: any employer can find a future doctor among high school students, then he applies to the university with a proposal to conclude an agreement on targeted reception indicating the number of citizens and areas of training in which specialists are required in his institution.

Does the USE result of this boy or girl matter?

The results of the Unified State Examination, if you go down to the bottom bar, must meet the requirements that Rosobrnadzor makes for the concept of a “troika”. Those. if the applicant has a complete secondary education and USE results, corresponding to the level established by Rosobrnadzor, an employer, for example, from a rural area applies to us, we must allow this applicant to participate in the competition. But since the university has a worthy image, socio-political significance, we, by decision of the academic council, raised the minimum score for the Unified State Examination in each subject, for example, in the specialty "Dentistry", from 36 to 50. Each subject in the Unified State Examination must be at least 50 points. In addition, the sum of points for the target recruitment at the Faculty of Medicine, I think, will be at least 200, for Pediatrics - 170-175. Our task is to make a filter so as not to take applicants with low basic training.

In addition, we transferred 124 places to the Ministry of Health of the region in 4 faculties - medical, pediatric, dental and medical cybernetics so that they would already recruit future doctors for especially understaffed institutions. The Ministry of Health has created a special commission, which included deputies of the Legislative Assembly of the Territory, representatives of the public, and this commission has set its bar - grades in the certificate are not lower than four. Despite the fact that there is a need for personnel, that the law obliges, we deliberately set two bars in order to weed out weak applicants - a total score of 150 and certificate indicators of 4.0.

How will now the “targeted” be obliged to work in the hospital that sent them to study?

Now the "targets" sign two agreements - the first is signed between the medical university and the medical institution, this document determines the system of interaction between these organizations, the second (an agreement on targeted learning) - between the employer and the applicant. The employer is obliged to accept students for practice, pay for travel. The government obligated the employer to provide social support measures. For example, in Khakassia, a republican law was adopted on the payment of 2,000 rubles a month to all students sent to a medical university on a targeted basis as a second scholarship. Unfortunately, in the Krasnoyarsk Territory, neither the Ministry of Health nor the acting governor responded to my appeal. For 124 people, this is only three million rubles a year, a small amount, but for a student this is an additional incentive. It was decided that each medical institution should find such funds on its own, but I see from the contracts that individual institutions from their paid services will provide such support. Therefore, I appeal once again to the leadership of the region and the Minister of Health: social support such a plan for students is necessary!

How many years will a young doctor have to work in a state medical institution?

It's just a contractual relationship. A minimum of three years, excluding internship and residency. But I see in the contracts even five years, and the FMBA has been writing for 10 years already.

Ivan Pavlovich, they are unsuccessfully trying to solve the personnel problem in medicine last years, and all characters shifting responsibility to each other, and there are fewer and fewer doctors in the industry. Can you name specific districts of the Krasnoyarsk Territory, medical institutions that send you “targeted” applicants who are really trying to solve the personnel problem in medicine?

We have about a dozen progressive heads of district administrations who are really doing something in this direction. Understand, it all depends on the position of the head of the territory. Let's take the Severo-Yenisei donor region, where the head is Ishmurat Gainutdinov. For many years, in addition to target places, it annually pays for the education of students, interns for specific children from the Severo-Yenisei region who return to work at home.

Are they returning?

Of course, they return, because they come, and the apartment is already ready for them. They arrived in July, so they even plant potatoes, they allocate a site, and therefore there is practically no problem with medical personnel in the North Yenisei region. The second example is the subsidized Nizhneingashsky district. The head, Petr Alexandrovich Malyshkin, has been coming to us since January with a list of applicants and students who will return to the district to work. They pay an additional stipend of 2,000 rubles to the children sent from Nizhny Ingash and provide housing. Two different districts with different financial conditions, but the heads are smart, progressive and far-sighted, so you can look up to them. There are no excuses related to finances, it all depends on the position of the leader. We have only 395 budget places We are recruiting 1,000 freshmen. Kansk, Achinsk choose places well, there are regional programs there.

I have been trying to promote a law in the Krasnoyarsk Territory on educational loans for a long time. Conditionally, the region orders 100 places from us at the Faculty of Medicine, pays for tuition, and graduates after graduation are required to work in a municipal medical institution. This is the most optimal solution to the personnel problem. If you don’t want to work, return the money spent on your education to the state. By the way, this is now spelled out in the law: if a student who has studied according to the target set does not go to the countryside, he will be obliged to return twice more money spent on his education. Imagine if we had adopted a law on educational loans - every year 100 new doctors would arrive in rural areas, in urban medical institutions. I believe that for the new governor of the Krasnoyarsk Territory, solving the personnel problem in medicine is the number one task.

Ivan Pavlovich, in your opinion, how does the level of applicants change from year to year?

From my point of view, a basic level of knowledge of applicants becomes higher. And I can confirm this with a smaller number of deductions. Even 10 years ago, at the first sessions, we had a massive expulsion, now there are only a few. Basic education has grown, with the exception of chemistry, we see that chemistry is taught poorly at school - this year the USE in chemistry is 6 points lower than in the past, in biology it is 4 points lower, and one and a half in the Russian language.


As statistics show, the USE for girls is usually higher than for boys, while in medicine, especially in some specialties, male doctors are much more preferable. Is there such a problem as a reduction in the number of young people among freshmen?

Yes, there is such a problem, I began to analyze the situation, when at a meeting a student voiced the problem to me - there are few guys at the university, there is no one to marry. Indeed, girls study better, the USE has become the main element of passing, and we have a sharp imbalance in admission. The only way we can help is to treat the guys more condescendingly in the first sessions and not expel them. As a rule, a fracture in guys occurs in the third year, they begin to study better and become good doctors. But the disproportion is terrible! For example, at the pediatric faculty, 100 people graduate, 10 percent of them are guys. When I was a student, we had 217 graduates, and the ratio of men to women was 50 to 50. Now only the Faculty of Dentistry maintains these proportions.

And the last question, if someone offers an applicant or his parents to enter a medical university for money, I mean a bribe ...

And no one gives bribes now, because it makes no sense. If you have money, bring it to the university cash desk 50 thousand per semester, score 50 points in the exam and do it calmly. We try not to expel anyone in the first semesters. Let them pass 3-4 times, but pass and study further, pass fundamental sciences because it is easier to study in the clinic. We give a chance, and in the third session we pass through the filter, and there is already a tough selection. Therefore, on the off-budget, instead of a bribe, I paid to the cashier, if you have a desire, then we can give knowledge and a person has a chance to become a doctor.

The acceptance of documents to the Krasnoyarsk State Medical University will end on July 25, 2014. Then we will be able to take stock and understand how prestigious medical education is now considered.

1. General Provisions

1.1. This Regulation regulates the activities of the department clinical residency and internships (hereinafter referred to as the department) of the Institute of Postgraduate Education of the State Educational Institution of Higher Professional Education "Krasnoyarsk State medical Academy federal agency health and social development(hereinafter IPO KrasGMA).

This Regulation has been developed in accordance with the Federal Law "On Higher and Postgraduate Professional Education", a resolution of the State Committee Russian Federation on higher education dated 04/10/1996 No. 2 "On approval of the model regulation on structural divisions of the additional vocational education(advanced training) of specialists organized in higher and secondary specialized educational institutions of the Russian Federation" and the regulation on the Institute of Postgraduate Education, approved by the Academic Council on October 26, 2006.

11. Purpose and objectives

2.1. The department of clinical residency and internship is being created as a structural subdivision of the IPO KrasGMA in order to improve the practical training of graduates of higher medical institutions, improve their professional level and their readiness for independent medical practice. Training of highly qualified specialists for independent work in the health authorities and institutions of the Krasnoyarsk Territory, the Republics of Khakassia, Tyva or in the order private practice.

2.2. The main tasks of the department of clinical residency and internship are:

Organization and conduct of training of specialists with medical education in internship and clinical residency (postgraduate education);

Organization of work to update and deepen the theoretical and practical knowledge and skills of specialists with medical education in connection with the increased requirements for the level of qualifications and the need to master modern methods solving professional problems of practical healthcare;

Organization of work to meet the needs of specialists in obtaining knowledge about the latest achievements in relevant areas of medical science and practice, based on advanced domestic and foreign experience;

Expertise curricula, guidelines and other materials on the profile of the department;

111. Functions of the department

In accordance with the assigned tasks, the department performs the following functions:

3.1. Organization of training of specialists with higher medical education in internship and clinical residency in accordance with the current nomenclature of specialties of medical workers;

3.2. Registration of an application for the training of doctors in clinical residency and internship in coordination with the health authorities of the Krasnoyarsk Territory, Khakassia and Tyva.

3.3. Participation in the process of organizing the preparation of training and production plans, lecture schedules for clinical residents and interns;

3.4. Registration in accordance with the established procedure of the necessary documents received in residency and clinical residency;

3.5. Preparation of orders for enrollment (expulsion) of interns and clinical residents;

3.6. Providing information to interns and clinical residents about the holding of approved lectures;

3.7. Preparation and certification of clinical residents at the departments;

3.8. Preparation of analytical materials and preparation of reports on the work of the internship and clinical residency;

3.9. Preparation of reporting documentation, both planned (quarterly, annual, statistical reports), and emergency, for others structural divisions KrasGMA, Ministry of Health and Social Development of the Russian Federation, Roszdrav;

3.10. Preparation of documents for correspondence with other organizations and institutions on internship and clinical residency;

3.11. Participation in the organization of a quality control system and testing the knowledge of interns and clinical residents at various levels, including during the final certification.

3.12. Participation in the organization of the quality management system educational process in the IPO.

3.13. Participation in the organization and work of international projects.

3.14. Organization of registration of documents to be stored in the archive.

1V. Organization of department activities

4.1. The department of clinical residency and internship carries out its activities in cooperation with other structures of the Institute of Postgraduate Education, KrasSMA, and health authorities.

4.2. The general management of the department is carried out by the head of the department of clinical residency and internship.

4.3. The staffing of the department - the head of the department, specialist and secretary;

4.4. The structure and staffing of the department is approved by the rector of KrasGMA;

4.5. The department uses material and technical, financial resources, personnel potential, clinical bases of departments and courses for the implementation of postgraduate education programs, which include: training in internship and clinical residency;

4.6. Admission to clinical internship is carried out on a competitive basis.

Training in clinical residency is carried out in accordance with the current orders of the Ministry of Health of the Russian Federation, the Ministry of Health and Social Development of the Russian Federation and the Regulations on Clinical Residency in accordance with educational standards by specialties.

Graduates of clinical residency who successfully complete the course of study are issued certificates of completion of residency of the established form and, in case of successful final certification, a certificate of a specialist.

4.7. Internship training is carried out in accordance with the current orders of the Ministry of Health of the Russian Federation, the Ministry of Health and Social Development of the Russian Federation and the Regulation on one-year specialization (internship) of graduates medical schools.

Graduates of the internship who successfully complete the course of study are issued certificates of completion of the internship of the established form and, in case of successful final certification, a certificate of a specialist.

4.8. The department, within its competence, interacts with the health authorities and institutions of the Krasnoyarsk Territory, and, if necessary, other territories of the Russian Federation.

4.9. The department, in accordance with the established procedure, reports on the results of its activities and submits annual statistical reports in the form No. 1-nk.

4.10. The remuneration of employees of the department is determined in accordance with labor contracts.

4.11. The activities of the department are carried out on the basis of work plans developed by the head of the department, approved by the vice-rector for postgraduate education - head of the Institute of Postgraduate Education, as well as official duties department employees.

4.12. Employees of the department bear the responsibility provided for by the legislation of the Russian Federation.

V. Rights of the department

In accordance with the assigned tasks and functions, the department has the right to:

5.1. Conduct independent business common system records management of the Institute of Postgraduate Education KrasGMA.

5.2. Request and receive Required documents and materials related to the activities of the department.

5.3. Enter into relationships with departments of third-party institutions and organizations to resolve operational issues of the production activities of the department.
